Gauge manufacturer, model, serial number, and last calibration date of the test equipment used.
Specific PSI readings for each check valve, relief valve opening point, and static line pressure.
Clear determination for each check valve and the overall assembly based on AWWA standards.
Certified tester name, certification number, certification expiration date, and signature.
